The Governing Board strongly supports a risk management program that protects district resources and promotes the safety of students, staff and the public.  The Superintendent or designee shall establish a risk management program that uses effective safety and loss control practices. 

The district shall strive to keep its liability at a minimum and its insurance premiums as low as possible while maintaining adequate protection.  To determine the most economical means of insuring the district consistent with required services, the Superintendent or designee shall annually review the district's options for obtaining coverage, including qualified insurance agents, a joint powers agency, self-insurance or a combination of these means.

The Board reserves the right to remove an insurance agent-of-record or a participating agent whenever, in the judgment of the Board, such action becomes desirable for the best interests of the district.  To attempt to minimize the district's exposure to liability, the Board shall adopt clear policies related to discrimination, harassment, safety procedures, and the timely handling of claims.  The Superintendent or designee shall ensure that these policies and related procedures are enforced fairly and consistently.

Employee Injury Reporting


EVERY INJURY must be reported on an  EMPLOYEE REPORT OF INJURY FORM .  The form must be given to your Supervisor and Risk Management.  However, this does not mean that you must see the doctor.  Risk Mgt will keep your Employee Report of Injury form on file should you need to seek treatment at a later date. Should you need to seek treatment, please do the following steps.

1. Report your injury to your Supervisor and complete an EMPLOYEE REPORT OF INJURY FORM - CLICK HERE FOR FORM . The form is provided here, and are also on file at your site.

2. Call Company Nurse at (877) 223-9307 and use Group Code "SIRMA" to report your injury. Do not see any doctor before reporting your injury to Company Nurse.  They are available 24 hours a day, 7 days a week.

3. Once you see the doctor, please do not report back to work until you have given Risk Management a copy of your doctor's note AND completed the additional time sensitive paperwork given to you by Risk Management.

A predesignation form must be on file with Risk Management should you wish to see your own physician.  For guidelines on who you may predesignate, contact the Risk Management Department.

Report Fraud!

workerscompfraud1.gifWhat is Insurance Fraud?  Fraud occurs when someone knowingly lies to obtain some benefit or advantage to which they are not otherwise entitled or someone knowingly denies some benefit that is due and to which someone is entitled. Insurance fraud wastes millions of dollars each year.  The California Department of Justice Fraud Division has established a method for insurers to report suspected insurance fraud. It is important to know that notification of insurance fraud may be made anonymously. USFA Releases Report Civilian Fire Injuries in Residential Buildings.
Report focuses on the causes and characteristics of fire injuries in residential buildings.
The United States Fire Administration recommends everyone should have a comprehensive fire protection plan that includes smoke alarms, residential sprinklers, and practicing a home fire escape plan.
 
May 2, 2012 Emmitsburg, MD. – The U.S. Fire Administration (USFA) issued a special report today examining the characteristics of civilian fire injuries in residential buildings. The report, Civilian Fire Injuries in Residential Buildings (2008-2010) (PDF, 277 Kb), was developed by USFA's National Fire Data Center and is based on 2008 to 2010 data from the National Fire Incident Reporting System (NFIRS).

According to the report:

  • Seventy-six percentof all civilian fire injuries occurred as a result of fires in residential buildings.

  • Cooking(30 percent) was the primary cause for residential building fires that resulted in injuries.

  • Thirty-five percent of civilian fire injuries in residential buildings resulted from trying to control a fire followed by attempting to escape (26 percent).

  • Seventy-nine percent of injuries resulting from residential building fires involved smoke inhalation and thermal burns.

  • The leading human factor contributing to injuries in residential building fires was being asleep (55 percent).

  • Bedrooms(35 percent) were the leading location where civilian injuries occurred in residential building fires.

►Posted October 21, 2011: Walgreens Pharmacy – Network Termination Effective January 1, 2012. Walgreens pharmacies will no longer be a viable option for most SISC plans. In most cases, a member using a Walgreens pharmacy as of January 1, 2012 will pay out of pocket for the entire cost of the drug.  We are concerned about the impact rising health care costs have on our members. The elimination of Walgreens helps in maintaining an affordable benefit program. We are committed to keeping your health care both affordable and accessible. Our analysis indicates 99.8% of current SISC Walgreens customers will be able to continue to travel the same distance or less to an alternative network pharmacy.

►POSTED OCTOBER 2011:  Health Smarts Digital Coaching - Earn up to $150!  Emailed to all AVUHSD employees October 18, 2011:  You and your family can participate if they are age 18 or older and are currently enrolled in a SISC health plan (Blue Cross PPO and California Care HMO).  You may log into http://www.sischealth.com and follow instructions. You may use your old username and password or if you’ve forgotten your username or password, there’s a link that you can click. Click HELP (upper right hand of the login page) if you need assistance or have questions. It may take up to 2 business days after you complete the health assessment, engage in digital health coaching, or complete a survey for your award status to be posted. After you complete the health assessment, it can take up to 2 weeks for your reloadable award card to be processed and mailed to you. This is handled by SISC, so district HR staff cannot access any info for you, check balance, check status of rewards, etc.  Kaiser members will have access to the programs, but will not be able to process the award until 11/1/11. When you log in, you will be automatically redirected to the Kaiser site. Just FYI that your October paycheck will reflect new health benefit rates and AFLAC (if you signed up for it)

►POSTED OCTOBER 2011: Free Flu Shot Clinic if you are enrolled in health benefits through the district, you are eligible to attend our free flu shot clinic being held only at the District office on October 26, 2011 from 10-5pm.  Make an appointment at www.siscflushot.org! This clinic is available to your spouse and/or dependents over the age of 18 as long as they are currently covered under your plan. Walk-ins on LIMITED BASIS. Appointments are STRONGLY ENCOURAGED to ensure an adequate supply of vaccine is available. For questions, please call 310 618 1988.

►POSTED AUGUST 2011: To all AVUHSD Employees: If you wish to make any changes to your health benefits plan, you must do so with the Risk Management Department NO LATER THAN SEPTEMBER 2, 2011. It is highly recommended that you do not wait until the deadline date!    The AVUHSD EMPLOYEE HEALTH FAIR will be held August 18, 2011.

►POSTED APRIL 2011:  SISC will be performing annual health assessments at our main sites during the month of April.  Participants will receive a $20 gift card. (Card is activated 2-3 days after your appointment) All employees of the district are able to attend the onsite screenings.  For those of you who have District insurance coverage, your spouse and or children over the age of 18 can also participate.   Please see the schedule below for dates and times. You may choose to attend any site.  You must register at the link below.  Online appointments are open 24-7 including weekends and will close at 3PM the day before each event.  Apologies, Risk Management is unable to help you with website issues including user names and or passwords.   Screenings: Fasting recommended but NOTrequired.  Drink plenty of water.  Black coffee without cream and sugar is considered fasting.  Screenings will take about 15-20 minutes per person and include: Blood Pressure,  BMI/Body Fat, Waist Measurement, Hip Measurement, Glucose, Total Cholesterol, TC/HDL Ratio  Note: One fingerstick for glucose and cholesterols.
