Employee Injury Reporting
EVERY INJURY must be reported on an EMPLOYEE REPORT OF INJURY FORM . The form must be given to your Supervisor and Risk Management. However, this does not mean that you must see the doctor. Risk Mgt will keep your Employee Report of Injury form on file should you need to seek treatment at a later date. Should you need to seek treatment, please do the following steps.
1. Report your injury to your Supervisor and complete an EMPLOYEE REPORT OF INJURY FORM - CLICK HERE FOR FORM . The form is provided here, and are also on file at your site.
2. Call Company Nurse at (877) 223-9307 and use Group Code "SIRMA" to report your injury. Do not see any doctor before reporting your injury to Company Nurse. They are available 24 hours a day, 7 days a week.
3. Once you see the doctor, please do not report back to work until you have given Risk Management a copy of your doctor's note AND completed the additional time sensitive paperwork given to you by Risk Management.
A predesignation form must be on file with Risk Management should you wish to see your own physician. For guidelines on who you may predesignate, contact the Risk Management Department.

Report Fraud!
What is Insurance Fraud? Fraud occurs when someone knowingly lies to obtain some benefit or advantage to which they are not otherwise entitled or someone knowingly denies some benefit that is due and to which someone is entitled. Insurance fraud wastes millions of dollars each year. The California Department of Justice Fraud Division has established a method for insurers to report suspected insurance fraud. It is important to know that notification of insurance fraud may be made anonymously. You may contact any of the Fraud Division Regional Offices directly responsible for your county. USFA Releases Report Civilian Fire Injuries in Residential Buildings.
Report focuses on the causes and characteristics of fire injuries in residential buildings. The United States Fire Administration recommends everyone should have a comprehensive fire protection plan that includes smoke alarms, residential sprinklers, and practicing a home fire escape plan.
May 2, 2012 Emmitsburg, MD. – The U.S. Fire Administration (USFA) issued a special report today examining the characteristics of civilian fire injuries in residential buildings. The report, Civilian Fire Injuries in Residential Buildings (2008-2010) (PDF, 277 Kb), was developed by USFA's National Fire Data Center and is based on 2008 to 2010 data from the National Fire Incident Reporting System (NFIRS).
According to the report:

Seventy-six percentof all civilian fire injuries occurred as a result of fires in residential buildings.

Cooking(30 percent) was the primary cause for residential building fires that resulted in injuries.

Thirty-five percent of civilian fire injuries in residential buildings resulted from trying to control a fire followed by attempting to escape (26 percent).

Seventy-nine percent of injuries resulting from residential building fires involved smoke inhalation and thermal burns.

The leading human factor contributing to injuries in residential building fires was being asleep (55 percent).

Bedrooms(35 percent) were the leading location where civilian injuries occurred in residential building fires.
